The CAM Benefit Program’s Prescription Drug benefits are administered by CVS/Caremark.

Retail Co-Payment Plans. Different drug plans have different co-pays. A member's co-pay amount(s) is available to the participating pharmacies via their computer system which is directly on-line with Caremark. A member can obtain the amount of a prescription drug co-pay by calling Caremark @800 777-1023 or on-line at www.caremark.com .

Most drug plans have a generic enforcement provision with a lesser co-pay applicable when a generic substitute is obtained. The designated co-payment will be charged to the member in addition to 100% of the cost difference between brand and generic when the physician allows a generic to be dispensed and the member has requested that brand be filled. If the physician indicated brand must be dispensed, a penalty will not be charged, but the brand co-pay will apply.

If a group’s prescription drug plan is a formulary (3-Tier) plan, the member will have three possible co-pay options. 1st Tier drugs provide the lowest co-pays by prescribing a generic drug. 2nd Tier drugs provide higher co-pays by prescribing a preferred brand drug. 3rd Tier drugs provide the highest co-pays by prescribing a non-preferred drug. A list of drugs should be included in the member's packet if he/she is enrolled in this type of prescription drug plan. Typically, Caremark makes modifications to its Preferred Drug List in the Spring and Fall each year. The member can also obtain a list of drugs from Caremark by calling 800 777-1023 or log on to www.Caremark.com  for an on-line preferred drug list.

Mail Order Prescription Service. A Mail Order Prescription service is also available. This service offers an inexpensive way to order long-term prescription medication that is taken for chronic illnesses such as diabetes, high blood pressure or any other covered prescription medication that is taken on an on-going basis. The advantages to mail order are convenience, lower costs and fewer trips to the drug store. Through mail order, a member can obtain a 90 day supply of his/her prescription with a lesser co-pay (instead of a co-pay for each 30 day supply).   • Go to www.Caremark.com 
  • Click on MY BENEFITS and complete registration
  • Click on “start using Mail-order”
  • Complete info on form and enter “submit” or print and fax/mail to number/address at top of the form
  • A Caremark Direct pharmacist will contact your physician to obtain a new prescription to start providing your medication through the mail service pharmacy.

Oral Contraceptives. This coverage is not included in all employer plans. You should contact your employer, your pharmacy, CAMADS or Caremark if you do not know whether you have this coverage.
